You know what’s funny? John looks like he’s realising something, like he just realised what Sherlock did.
Let’s consider the first modern scene as real (or partially real). So John already knows Sherlock “went to the trouble of an overdose”. Just as he knows what Mycroft says: he took the drugs before the tarmac scene - thus, before even knowing about Moriarty being apparently alive.
Therefore, John is realising Sherlock went to the troubles of an overdose for a very different reason.
Starting to put the pieces together John? Look the guilty face Sherlock did, he’s not exactly telling the truth, you know that John, you are realising now, don’t you? You begin to understand why, precisely, really, Sherlock drugged himself and OD’d, don’t you?
Usually we call that an epiphany.
